Important Dates to Remember

September 3, 2019 Physicals and Immunizations due

October 15, 2019 Vision exams due for Kindergarten and Students entering in an Illinois school for the first time from out of state.

May 15, 2020 Dental Exams due for Kindergarten, 2nd and 9th graders

Attention All Upcoming Kindergarten, 6th, 9th and 12th Graders

Those students entering Kindergarten, 6th,9th and 12th grades in 2016 are required to have an Illinois school physical and/or Proof of immunizations.

2 meningococcal unless the first one is given on or after the age of 16

The DHS School physical form and proof of immunizations is due September 3, 2019 or before. THIS IS ABSOLUTELY REQUIRED! Do not get a sport’s physical this summer because your school physical can be used as a sport’s physical. This will save you $20 and a lot of trouble.
