All new students coming from out of state, regardless of age or grade, will need the following:
IL physical exam
Proof of immunization
IL dental exam
IL vision exam
Important Dates to Remember:
October 1st of each school year: Physical Exam and Immunizations due
October 15th of each school year: Vision exams due for students in Kindergarten and students entering an Illinois school for the first time.
May 1st of each school year Dental Exams due for Kindergarten, 2nd and 6th graders

Those students entering Kindergarten and 6th grades are required to have an Illinois School Physical Exam and Proof of immunizations, specific information is listed below.
